What to Expect During Your Consultation

Your consultation is a detailed planning session. During this visit, you will:

• Receive precise measurements of your chest wall dimensions, tissue thickness, and skin elasticity to map a sizing range specific to your frame.

Incision, Placement, and Surgical Techniques

Your surgeon selects incision and placement methods based on your implant size, anatomy, and target outcome.

• Inframammary fold incision (placed in the natural crease under the breast) is the preferred approach for extra large breast implant surgery. It provides the widest access for placing high-volume devices and heals as a thin line concealed beneath the breast.
• Periareolar incision (around the areola border) may be considered for implant sizes at the lower end of the XL range in select cases, depending on areola diameter and implant dimensions.
• Submuscular placement positions the implant beneath the pectoral muscle, adding a supportive layer that distributes implant load more evenly across the chest wall and reduces visible edges.
• Dual-plane positioning (implant placed partly under muscle and partly under breast tissue) takes this a step further: the upper portion of the implant sits behind the muscle for contour and coverage, and the lower portion rests beneath the breast tissue for a natural drape. This is the most common approach for XL volumes.

The procedure is performed under general anesthesia at our San Diego surgical facility. It typically takes 1.5 to 2 hours, and you can return home the same day with detailed recovery instructions and a dedicated care team available to answer questions.

Internal Bra Support With GalaFLEX Mesh

Extra large implants place a significant mechanical demand on breast tissue. To address this, your surgeon often uses GalaFLEX mesh to create an internal bra, a soft tissue scaffold that reinforces the lower pole of the breast and supports the weight of high-volume implants from within.

GalaFLEX is a flexible, FDA-cleared surgical mesh made from poly-4-hydroxybutyrate (P4HB), a bioabsorbable material with a strong safety record across surgical applications. During your procedure, the mesh is anchored to the chest wall and shaped to act as a hammock beneath the breast tissue. Over the next 12 to 18 months, your body gradually absorbs the scaffold and replaces it with your own collagen, leaving behind a layer of reinforced tissue that continues to support the implant long after the mesh is gone.

For XL augmentation patients, the internal bra technique offers several benefits:

• Stabilizes the implant pocket and helps prevent bottoming out under the weight of high-volume devices
• Reinforces thin or stretched tissue in patients with limited natural soft tissue coverage
• Reduces strain on the skin envelope during early healing by distributing the implant load
• Supports long-term shape and projection by encouraging natural tissue strengthening over time

The internal bra is not used in every case. Your surgeon evaluates your tissue quality, implant size, and anatomy during your consultation to determine whether GalaFLEX would meaningfully add to your result.

Recovery After Extra Large Breast Augmentation

Recovery after high-volume augmentation requires more patience and structured support than a standard-sized procedure. Early on, your implants may appear too high or too round. Over three to six months, each implant drops into its intended position, creating a more balanced upper and lower pole and reaching its final shape.

Following your recovery plan closely makes a measurable difference in your final shape.

• Days 1 to 3: Rest at home. Wear your surgical bra continuously. Expect tightness, swelling, and moderate discomfort managed with prescribed pain medication. Your implants will sit high on the chest initially.
• Week 1: Light walking promotes circulation and prevents blood clots. Incisions stay covered with steri-strips or surgical tape. Most patients manage discomfort with anti-inflammatory medication by day five.
• Weeks 2 to 4: Swelling peaks around week 2 and begins to subside. Light daily activities resume. No lifting anything over five pounds. Continue wearing the surgical bra around the clock.
• Weeks 4 to 6: Transition to a supportive sports bra. Dr. Chao removes the incision strips based on your healing progress. Your implants begin settling into a lower, more natural position as gravity and tissue relaxation take effect.
• Week 6 and beyond: You may be cleared for exercise and full physical activity. Swelling continues resolving over the following months. Settling takes longer with extra large breast implants because of the added implant volume.

Long-Term Breast Implant Maintenance
